There's a whole lot of time killing going on. Serenity is heading toward Ariel, a core planet and Wash wants to go sightseeing, Jayne's cleaning various weaponry and Simon is trying to convince River that the slop he's cooking tastes good. Suddenly River picks up a knife and slices Jayne across the chest, declaring he "looks better in red." Simon stiches up Jayne in the infirmary and Mal warns Simon to keep his sister in check. Later on he comes in and tells Mal and Jayne that he has a job for them. 

The idea he has is to steal a ton of medicine from the Alliance-run hospital. Reselling the drugs will bring in a small fortune. So, now that Simon has a plan, the crew gets to work on bringing to fruition his method of getting them into the hospital. That includes Wash and Kaylee getting an ambulance shuttle made over from a scrap heap. It also means Simon has to give Mal, Zoe and Jayne a crash course in medical jargon so they can bluff their way through the front door. They'll be bringing in Simon and River, who will be posing as corpses.

They get in successfully and Jayne makes contact with the Alliance official who he plans to turn Simon and River in to. The two wake up from their induced sleep and Jayne walks with them to the examining room where Simon can run a full battery of tests on River. Mal and Zoe are off to steal the medicine and run into a little trouble, but nothing they can't handle. After Simon examines River for a few moments Jayne gets them moving, saying the plan has changed. He leads them out a back way where a full squad of Alliance officials are waiting for them.

The three are arrested as Mal and Zoe hook back up with Wash at the shuttle. After the planned rendevous time has passed Mal realizes the other three have probably been nabbed. Simon, River and Jayne manage to escape the Alliance folks. While they're running two men wearing blue gloves show up. They're the ones who were going to collect the fugitives and, after finding out the Alliance captain spoke to them, they pull out a stick that causes his nose and eyes to bleed and eventually kills him. River is freaking out because of their presence.

Eventually they all meet up and Wash gets the gang back on Serenity via the shuttle. As they're getting the medicine stowed away Mal hits Jayne with a wrench and drops him on the bad side of the airlock with a communicator. He knows it was Jayne that tipped off the feds and Mal informs him that because he ratted on a member of Mal's crew he takes it very personally. Jayne pleads for his life and finally asks Mal to not tell the others what he did after he's dead. Mal finally closes the cargo bay door and spares Jayne for the time being.

Finally, Simon brings River some medicine declaring that for her, "it's time to wake up."
